- Must have selected FUHSA as first choice institution.
- Must possess at least five relevant O’Level credit passes, including:
- English Language
- Mathematics
- Physics
- Biology
- Chemistry
- O’Level credits must be obtained in not more than two sittings.
- Must have sat for the appropriate UTME subject combination required for the chosen programme.

Courses Requiring 200 and Above
Candidates applying for the following programmes must score at least 200:

Courses Requiring 160 and Above
Candidates applying for the following programmes must score at least 160:
- Nutrition and Dietetics
- Information Technology and Health Informatics

- Occupational Therapy
- Health Information Management
- Public Health
- Pharmacology
Courses Requiring 150 and Above
Candidates applying for the following programmes must score at least 150:
