- UI Authority-To-Pay (colour printed) and a photocopy [to be printed as from Tues., 4/6/2024]
- FORM B003
- Admission Letter (Institutional copy) (colour printed)
- JAMB Result Slip
- O/Level result (2 copies)
- Two (2) Reference Letters from reputable personalities (on letterheaded paper) addressed to the Registrar, University of Ibadan
- Birth Certificate (photocopy)
- Four (4) passport photographs
- A file and tags
