Mater Misericordiae Hospital Afikpo 2024/2025 supplementary admission into basic Midwifery; This serves as a public announcement regarding applications for admission into the 3-year Basic Midwifery Programme at the School of Midwifery, Mater Misericordiae Hospital, Afikpo.

ENTRY REQUIREMENTS:

1. Applicants must be a minimum of 16 years old.
2. Candidates must hold senior secondary school certificates or their equivalents (SSCE, NECO, NABTEB, GCE) with a minimum of five (5) credits in English language, Mathematics, Physics, Chemistry, and Biology, obtained in not more than two sittings.
3. Birth certificates or declaration of age.
4. Testimonial from the last school attended.
5. A letter from the parish priest or pastor.
6. Marriage certificate (if applicable).
7. Original and photocopies of relevant documents will be required during the interview.
8. Three (3) recent passport photographs.
9. Baptismal card (mandatory).
10. Any of PVC, NIN, International Passport, or Drivers license.

METHOD OF APPLICATION:

1. Forms are available for purchase at the school until March 20, 2024.
2. Obtain the form upon payment of a non-refundable fee of N12,550 into the school’s account with the following details:
   • Account Number: 3031304862
   • Account Name: School of Midwifery Mater Hospital, Afikpo
   • Bank: First Bank Of Nigeria PLC.
3. Visit the school with proof of payment to exchange the bank teller for a receipt and collect the entrance form.
4. Completed forms must be returned to the school on or before March 21, 2024.
5. The entrance examination will cover Mathematics, English Language, Physics, Chemistry, and Biology.

DATE OF EXAMINATIONS/INTERVIEWS:

March 21, 2024. Candidates must bring the original receipt of payment. Only those meeting the entry requirements and passing the entrance examination will be eligible for the interview.
